What is PACE?

All-inclusive care to help older adults live their best lives at home.

PACE of Cincinnati, a Program of All-Inclusive Care for the Elderly (PACE), is a Medicare and Medicaid program that helps people meet their health, social and long-term care needs in the community instead of going to a nursing home or other care facility.

When you join PACE, you’ll work with a local team of health care professionals who get to know you and who are dedicated to meeting your unique needs.

Our goal is to help you live your best life at home.

How Does PACE Work?

PACE provides all Medicare- and Medicaid-covered care and services, and anything else the members of your care team decide you need to improve and maintain your health. This includes prescription drugs and any medically necessary care.

When you enroll, PACE of Cincinnati becomes the sole source of services for Medicare and Medicaid eligible enrollees. An interdisciplinary team (care team) provides and coordinates health care and other services and works hand-in-hand with you and your family to develop a personalized care plan. Our team of professionals is experienced in working with older adults.

When you enroll in PACE, you must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from PACE of Cincinnati. PACE of Cincinnati becomes your sole service provider, guaranteeing access to services, but not to a specific provider.

Who is Eligible for PACE?

PACE of Cincinnati is available to Hamilton County residents who meet the following criteria:

  • Age 55 or older
  • Hamilton County resident
  • Eligible for nursing home care
  • Able to live safely in the community with support from PACE of Cincinnati services

Even if you don’t have Medicare or Medicaid, you may still be eligible for PACE.

What Does PACE Cost?

PACE allows participants to use both Medicare and Medicaid benefits to access and pay for services. Once you enroll in PACE, there are no benefit limits, deductibles or copayments for any services provided or coordinated by your PACE of Cincinnati care team.

Whether you have a premium depends on your financial situation:

  • If you qualify for Medicaid or are dually-eligible for Medicare and Medicaid, you pay no premium.
  • If you qualify for Medicare only, you pay the Medicaid portion, plus a monthly premium for Medicare Part D.
  • If you are not eligible for Medicaid or Medicare, you can pay for PACE yourself through a self-pay premium.

If you live in or move into an assisted living or skilled-nursing facility, you may be responsible for a copay.

PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
