PACE covers all Medicare Parts A, B and D benefits, all Medicaid-covered benefits, and any other services or supports that are medically necessary to maintain or improve the health status of PACE program participants.
Our PACE of Cincinnati care team will get to know you and will work with you and your family to create an individualized care plan. Any services on your care plan are included in and covered by the PACE program. Click here for more information about PACE services.
When you enroll, PACE of Cincinnati becomes the sole source of services for Medicare and Medicaid eligible enrollees. Our care team provides and coordinates health care and other services and works hand-in-hand with you and your family to develop a personalized care plan. Our team of professionals is experienced in working with older adults.
As a PACE participant, you must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from PACE of Cincinnati. PACE of Cincinnati becomes your sole service provider, guaranteeing access to services, but not to a specific provider.
If you have Medicare and Medicaid, you may not have to pay anything for PACE services. If you have only Medicare, you will have to pay the Medicaid portion of the cost of PACE. Your cost for services will be determined during the enrollment process.

The PACE model of care is based on an integrated team of professionals (doctors, nurses, social workers, therapists and more) who work together and with you to coordinate all of your care needs. This model provides complete care coordination, allowing PACE of Cincinnati to be a “one-stop shop” for all your care needs – from health care to help with daily activities. If you are concerned about whether you will be able to continue to see your current physician, please let us know during the enrollment process.
A typical day at PACE of Cincinnati starts with being picked up at your home by a PACE driver. When you arrive at the PACE center in Norwood, you will be helped into the building by PACE staff. You may stay at the PACE center for the whole day and participate in recreational activities, meals and medical appointments, or you might just stop in for a visit to the physician, occupational therapist or physical therapist. Regardless of how frequently you visit the center or how long you stay, we provided door-through-door transportation for your trip.
Yes, PACE of Cincinnati provides transportation to and from the PACE center and other medically necessary appointments. We have a variety of vehicles to meet participants’ needs. Our vehicles are accessible and can accommodate wheelchairs and other assistive devices. Door-through-door service is provided.
While PACE of Cincinnati only provides services to Hamilton County residents, there are PACE organizations across the country. Visit the National PACE Association website to find other PACE programs and contact them directly. You may also use the national Eldercare Locator to learn about and connect to programs and services for older adults in your new community. No, once you are enrolled in PACE of Cincinnati, your care will be covered unless you disenroll from the program.
