PACE Services

All-inclusive care for older adults.

PACE covers all Medicare Parts A, B and D benefits, all Medicaid-covered benefits, and any other services or supports that are medically necessary to maintain or improve the health status of PACE program participants.

We understand that everyone is different and has unique care needs. As a PACE of Cincinnati participant, you’ll have a team of professionals dedicated to meeting your medical, physical and emotional needs. The PACE of Cincinnati care team gets to know you and works with you and your family to create a personalized care plan designed to help you live your best life at home.

PACE of Cincinnati provides services across all care settings, 24 hours a day, 7 days a week:

  • At the PACE of Cincinnati day health center and via our network of local providers, participants receive primary care, therapy, meals, recreation, socialization, personal care and more.
  • In the home, PACE offers skilled care, personal care supportive services, and supports such as ramps, grab bars, and other tools that help participants live safely at home.
  • In the community, PACE offers access to local specialists and other providers.
  • Transportation is provided to the PACE of Cincinnati day health center and to all PACE appointments, regardless of location.
  • We’re here when you need us. For PACE of Cincinnati participants and their caregivers, our care team is always only a phone call away, 24 hours a day, seven days a week.

When you enroll, PACE of Cincinnati becomes the sole source of services for Medicare and Medicaid eligible enrollees. Our care team provides and coordinates health care and other services and works hand-in-hand with you and your family to develop a personalized care plan. Our team of professionals is experienced in working with older adults.

As a PACE participant, you must receive all needed health care services, including primary care and specialist physician services (other than emergency services), from PACE of Cincinnati. PACE of Cincinnati becomes your sole service provider, guaranteeing access to services, but not to a specific provider.

Day Health Center

Day Health Center

A safe and supportive environment for socialization, meals, activities and medical care. The day health center supports the needs of participants who have Alzheimer’s disease and other forms of dementia, offering a stimulating, supportive environment and peace of mind for family caregivers. Transportation is provided to and from the day health center.

Dental Care

Dental Care

Routine dental exams, including regular cleanings, dental procedures and dentures. Transportation is provided to and from PACE dental providers.

Healthy Meals

Healthy Meals

Nutritional counseling and meals. Meals are provided at the PACE center and/or via home-delivered meals.

Hospital Care & Emergency Services

Hospital Care & Emergency Services

Emergency room visits, hospitalizations, inpatient specialist care and skilled inpatient rehab services are covered by PACE.

In-home Care

In-home Care

Skilled nursing care, personal care, housekeeping and home safety supports.

Medical Care

Medical Care

Primary and specialty medical care, lab tests and diagnostics. Services are provided at our day health center and via our network of local service providers. Transportation is provided to all PACE appointments.

Prescriptions and Over the Counter Medications

Prescriptions and Over the Counter Medications

All medically necessary medications without co-payments. PACE participants do not need a separate Medicare drug plan.

Social Services

Social Services

Social work counseling, transportation and access to community resources.

Therapies

Therapies

Physical, occupational and recreational therapy. Therapies are provided in our day health center and via our network of local service providers. Transportation is provided to all PACE appointments.

Transportation

Transportation

Transportation is provided for all PACE-related appointments, including to and from the PACE of Cincinnati day health center. PACE vehicles are fully accessible and drivers ensure safe travel by providing door-through-door assistance and accommodation for assistive devices.
