About PACE of Cincinnati
PACE of Cincinnati offers personalized, coordinated and comprehensive care to Hamilton County older adults with the goal of helping them remain independent in their homes. Our interdisciplinary team (care team) of health and social service professionals provides and coordinates care for PACE participants in a variety of settings, including at our PACE of Cincinnati day health center, in provider offices and in participants’ homes.
