As a PACE of Cincinnati participant, you’ll have a team of local professionals dedicated to meeting your medical, physical and emotional needs. The PACE of Cincinnati Interdisciplinary Team (care team) gets to know you and works with you and your family to create a personalized care plan designed to help you live your best life at home.
